Our case management program is grounded in Hawaiʻi’s unique cultural values and an ʻohana-centered approach. We walk alongside clients as they navigate complex systems of care—connecting them to essential services, coordinating with healthcare and social service providers, and helping remove barriers to stability and wellness.
Each plan of care is holistic, respectful, and designed to meet clients where they are—at home, at school, and in the community.
